THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Yes I Doula LLC (“Yes I Doula,” “we,” “us,” or “our”) understands that information about your health and health care is personal. We are committed to protecting the privacy and security of your protected health information (“PHI”).
We create and maintain records regarding the doula care, services, care coordination, communications, and other services you receive from us. We use these records to provide and coordinate services, operate our organization, obtain payment when applicable, meet contractual and regulatory requirements, and comply with applicable law.
This Notice describes your rights, our responsibilities, and the ways in which we may use and disclose your health information.
YOUR RIGHTS
You have certain rights regarding your health information.
Get an Electronic or Paper Copy of Your Health Record
You may ask to see or obtain an electronic or paper copy of your doula care record and other health information we maintain about you.
Contact us to request access to your records. We will generally provide a copy or summary of your health information within 30 days of your request, as required by law. We may charge a reasonable, cost-based fee when permitted by law.
Ask Us to Correct Your Health Record
You may ask us to correct health information about you that you believe is incorrect or incomplete.
We may deny your request in certain circumstances, but if we do, we will explain the reason to you in writing, generally within 60 days.
Request Confidential Communications
You may ask us to communicate with you in a particular way or at a particular location.
For example, you may ask us to contact you at a particular telephone number, email address, mailing address, or through another available secure communication method.
We will accommodate reasonable requests.
Ask Us to Limit What We Use or Share
You may ask us not to use or disclose certain health information for treatment, payment, or health-care operations.
We are not always required to agree to your request. For example, we may decline a restriction if it would interfere with appropriate care or operations.
If you pay for a service completely out of pocket, you may ask us not to disclose information about that service to your health plan for purposes of payment or health-care operations. We will honor that request unless disclosure is required by law.
Get a List of Certain Disclosures
You may ask for an accounting of certain disclosures of your health information made during the six years before the date of your request.
The accounting generally does not include disclosures made for treatment, payment, or health-care operations and certain other disclosures excluded by law.
We will provide one accounting during any 12-month period at no charge. We may charge a reasonable, cost-based fee for additional requests during the same 12-month period.

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

Choose Someone to Act for You
If you have authorized someone to act on your behalf, such as through a medical power of attorney, or if someone is your legal guardian or otherwise legally authorized to act as your personal representative, that person may exercise your privacy rights.
We will verify the person's authority before allowing them to act on your behalf.
File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Yes I Doula LLC by contacting our Privacy Officer:

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
We will not retaliate against you for filing a complaint or exercising your privacy rights.
HHS specifically requires covered providers to explain these individual rights and provide a mechanism for complaints. HHS.gov
YOUR CHOICES
For certain health information, you may tell us your preferences regarding how we use or disclose your information.
You may tell us your preferences regarding:
Sharing information with your partner, family members, birth support persons, or others involved in your care.
Sharing information in a disaster-relief situation.
How we communicate with you regarding appointments, scheduling, follow-up, and services.
Certain marketing or fundraising communications, if applicable.
If you are unable to tell us your preference—for example, because you are unconscious—we may disclose relevant information if we reasonably believe doing so is in your best interest and the disclosure is otherwise permitted by law.
We may also disclose information when necessary and permitted by law to prevent or lessen a serious and imminent threat to health or safety.
Marketing and Sale of Information
We will not sell your protected health information.
We will obtain your written authorization before using or disclosing your protected health information for purposes requiring authorization under applicable law, including certain marketing activities.
If you give us written authorization, you may revoke that authorization in writing at any time, except to the extent that we have already acted in reliance on it.
OUR USES AND DISCLOSURES
How We Typically Use or Share Your Health Information
Provide and Coordinate Your Care
We may use your health information and disclose it to health-care professionals and others involved in providing or coordinating your care when permitted by law.
For example, when appropriate, relevant information may be shared with a physician, midwife, hospital, health plan, or another member of your care team to coordinate services.
Coordinate Doula Services
Yes I Doula uses a coordinated model of doula care. Depending upon the services you receive, members of your authorized care team may include prenatal doulas, birth doulas, postpartum doulas, care coordinators, administrative personnel, and other individuals involved in providing or supporting your services.
We may use and disclose the minimum information reasonably necessary among authorized individuals to coordinate your care, schedule services, facilitate continuity of care, document services, and support your pregnancy, birth, and postpartum needs.
Run Our Organization
We may use and disclose your health information to operate Yes I Doula, improve the quality of our services, coordinate staffing and coverage, train appropriate workforce members, perform quality-improvement activities, maintain records, conduct compliance activities, and contact you when necessary.
For example, authorized personnel may review documentation to ensure that doula services meet our quality and documentation standards.
Bill for Your Services
We may use and disclose your health information to obtain payment for services.
For example, when Yes I Doula participates with a health plan or other payer, we may provide information necessary to verify eligibility, obtain authorization when required, submit claims, document services, respond to requests for information, or support payment.
This may include disclosures to Medi-Cal managed care plans, commercial health plans, government health-care programs, or other payers with whom we participate.
Health Plan, Regulatory, and Contractual Oversight
We may use or disclose health information as permitted or required for audits, credentialing, utilization review, quality assurance, program integrity, fraud-waste-and-abuse investigations, regulatory reviews, or other lawful oversight activities.
These activities may involve health plans, the California Department of Health Care Services, government agencies, auditors, or other authorized entities.
OTHER USES AND DISCLOSURES PERMITTED OR REQUIRED BY LAW
We may use or disclose your health information in other circumstances permitted or required by law.
Public Health and Safety
We may disclose health information for legally authorized public-health and safety activities, such as:
Preventing or controlling disease;
Reporting certain adverse events;
Reporting suspected abuse, neglect, or domestic violence when authorized or required by law;
Preventing or reducing a serious threat to health or safety; and
Other public-health activities permitted or required by law.
Research
We may use or disclose health information for research when the use or disclosure is permitted by applicable law and all required protections and approvals have been satisfied.
Comply With the Law
We will disclose health information when federal or state law requires us to do so and will limit the disclosure as required by applicable law.
Organ and Tissue Donation
When applicable and permitted by law, we may disclose health information to organizations involved in organ, eye, or tissue donation and transplantation.
Medical Examiners and Funeral Directors
We may disclose health information to coroners, medical examiners, and funeral directors when permitted or required by law.
Workers' Compensation, Law Enforcement, and Government Activities
We may disclose health information when permitted or required by law for:
Workers' compensation claims;
Certain law-enforcement purposes;
Health-oversight activities;
Correctional institutions or law-enforcement custody;
Special government functions; and
Other legally authorized government activities.
Lawsuits and Legal Proceedings
We may disclose health information in response to a valid court or administrative order, subpoena, discovery request, or other lawful legal process when permitted or required by law.
SUBSTANCE USE DISORDER RECORDS
To the extent that Yes I Doula receives or maintains substance use disorder patient records that are protected by 42 CFR Part 2, additional federal confidentiality protections may apply.
We will use and disclose such information only as permitted by applicable law.
Protected Part 2 information generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your written consent or an appropriate court order and subpoena as required by law.
HHS's February 2026 provider model now specifically requires covered health-care providers and health plans that create or maintain Part 2 records to address those records in their privacy notices. HHS.gov
OUR WORKFORCE AND CONTRACTED DOULAS
Yes I Doula may provide services through employees, independent contractors, contracted doulas, care coordinators, administrative personnel, and other authorized individuals.
Individuals working on behalf of Yes I Doula who are permitted access to PHI must access, use, and disclose that information only as necessary for their authorized responsibilities and in accordance with applicable privacy and security requirements.
Where required, Yes I Doula maintains appropriate agreements, confidentiality requirements, policies, safeguards, and other protections governing access to PHI by contractors, vendors, business associates, and other parties.
Some contracted providers may also have independent legal responsibilities concerning health information they create or maintain.
ELECTRONIC SYSTEMS AND COMMUNICATIONS
Yes I Doula may use electronic systems to maintain health records, coordinate services, communicate with clients, conduct telehealth or virtual visits, schedule appointments, submit claims, and perform other health-care and administrative functions.
We select and implement systems and safeguards appropriate to the information being handled and our legal obligations.
When a secure patient portal, electronic health record, or other health-care platform is made available, we may ask you to use that system for communications involving sensitive health information.
We may also communicate with you by telephone, email, or text message according to your preferences and applicable consent requirements.
Electronic communication methods can carry privacy and security risks. You may request reasonable alternative methods of communication.
CALIFORNIA PRIVACY PROTECTIONS
California law may provide privacy protections that are more restrictive than federal law for certain types of health information.
When California law provides greater privacy protection than HIPAA or other applicable federal requirements, Yes I Doula will follow the more protective requirement when applicable.
This may include additional protections applicable to certain categories of sensitive health information.
HHS instructs covered entities to account for state laws that impose more stringent restrictions on uses or disclosures of PHI. HHS.gov
OUR RESPONSIBILITIES
Yes I Doula LLC is committed to protecting the privacy and security of your protected health information.
We are required to:
Maintain the privacy and security of PHI as required by applicable law.
Provide you with this Notice explaining our legal duties and privacy practices.
Follow the duties and privacy practices described in the Notice currently in effect.
Notify affected individuals as required by law if a breach occurs that may have compromised the privacy or security of their information.
Honor your rights regarding your health information as required by law.
Obtain your written authorization before using or disclosing PHI when authorization is required.
We will not use or disclose your information other than as described in this Notice or otherwise permitted or required by law.
If you authorize us in writing to use or disclose your information, you may revoke that authorization in writing at any time, subject to applicable legal limitations.
CHANGES TO THIS NOTICE
We may change the terms of this Notice and our privacy practices.
Changes may apply to health information we already maintain as well as information we receive in the future.
When we make a material change, we will update this Notice and make the current version available on our website and upon request.
ORGANIZATION COVERED BY THIS NOTICE
This Notice applies to:
Yes I Doula LLC
and to the health-care and doula services provided through Yes I Doula LLC by its authorized workforce and other persons acting on its behalf, as applicable.
Yes I Doula LLC is an independent organization and is not currently participating in an Organized Health Care Arrangement (OHCA).
As our organization grows, Yes I Doula may contract with health plans, managed care organizations, government programs, hospitals, health-care organizations, doulas, and other providers to coordinate and provide services.
Participation in such arrangements does not by itself make those organizations part of Yes I Doula LLC or subject them to this Notice.
